Thomas Joly-Mischlich and Annick Dufour were awarded a 1st Place HOPE Award in Inter-Professional Programs to Support Patient Care for their project, ‘SARO-MAVO (Active monitoring of patient-reported outcomes in oncology – Orally administered antineoplastic drugs)’.
Thomas and Annick identified a gap in follow-up symptom monitoring for patients treated with antineoplastic medication administered orally (MAVO). They leveraged a pre-existing virtual platform, already in use in Quebec for COPD patients and high-risk pregnancies, for active symptom monitoring of MAVO-treated patients and managing side effects of multiple oral therapies in the living environment.
Design: Following 1 year of algorithm development by a small team of pharmacists, the SARO-MAVO pilot was launched in spring 2022 at 3 separate hospital centres. A steering committee ensured healthy communication between all teams during the development stages, including doctors, nurses, and pharmacists. The algorithms collected patient-reported outcomes (PRO) in relation to their symptoms for 1 year.
Results: During the pilot project, 227 patients used the platform and the alerts generated by the occurrence or worsening of a symptom generated 740 clinical interventions. Every 2-4 weeks, the steering committee submitted reports of the project progress to the Ministry, the directorates, and clinical teams to ensure alignment with stakeholders. Based on the positive pilot results, in fall 2023, the Ministry did a provincial launch of the free virtual monitoring platform.
This project allowed the oncology pharmacists to develop extensive expertise not only in the management of the side effects of MAVO, but also in the overall assessment of the progression of the disease. The results from the pilot project show that this type of comprehensive pharmaceutical care is possible, feasible and desirable in all types of environments.
